How to read these pages
Nursing homes are surveyed by the state on behalf of Medicare (CMS), which publishes every health deficiency with a plain-English description, a scope-and-severity grade from A to L, the fines it imposed and the full ownership trail. We show it as published: A to C is no harm, D to F potential for harm, G to I actual harm, J to L immediate jeopardy. We compare homes by deficiencies per survey, not by raw count, because a home surveyed ten times will always have more on paper than one surveyed twice.
Assisted living is licensed by Texas HHSC, which publishes the licence, the owner, the management company, Alzheimer certification and every closure in bulk, and the inspection findings through its provider portal one facility at a time. Where we have not yet pulled a facility’s findings the page says so. That is not a clean record, and we never show it as one.
The summary on every page is free. The full report, with every finding described and the correction history, is $12, once. Facility operators can claim their page; paid placements are labelled and never change the record.
